lmb: Rename _lmb_alloc_addr() to lmb_alloc_addr_flags()

lmb_alloc_addr_flags() is a wrapper for _lmb_alloc_addr() and it's the
only function using it. Rename _lmb_alloc_addr() to lmb_alloc_addr_flags()
and remove the wrapper.
